I am wondering if I sign up for this and receive it, will I have to rerun the tivonet setup again right after I receive it and reboot? Or is it just a patch that won't affect it?

IIRC, yes.

I think ANY software upgrade involves the wiping and rebuilding of the opposite partition, and it's subsequent use. Therefore, after the upgrade, you should have to redo the TiVoNET instructions on whichever the new partition on your hard drive is after the upgrade.
