Beginner:
I am planning to add a new hard drive (120gb) to my Directivo.
Is it better to add as a B drive or replace the current drive with the new drive?
If I add as a B drive, do I still have to backup the A (current) drive?
If I add as a B drive, do I have to worry about the swap problem?

i have the t60, and decided to replace my original HD.
It was WAY to loud. i dropped a 120 GB Seagate Barraccuda V. (well i am doing it as we speak, wish me luck)

You can add a 120Gb drive to a DTiVo without worry about swap - mfsfix (the 'green screen') will work on DTivoes up to about 180Gb.
I would recommend taking a backup of the A drive AND replacing it with the new drive.
If you want to save your recordings do an MFS Tools pipe, otherwise just do a backup/restore.
If you decide to add a B drive, use MFS Tool's mfsadd function rather than BlessTiVo.
Use MFS Tools 2.0 as described in New Hinsdale.
